Nominations for the 2008 Indiana Public Health Foundation, Inc.'s Tony and Mary Hulman Health Achievement Awards (Indiana's "Premier Health Awards,") and the Foundation's Excellence in Health Science Research Award are accepted from hospitals, other health care providers, governmental agencies, colleges and universities, and individuals.  There are seven award categories.  Please email helenscheibner@netscape.net tor request the forms you need.  The forms may be completed electronically, saved as Word documents, and attached to an email to return to helenscheibner@netscape.net.  Please review Criteria for nominations.
